As a Social Worker within our Bridging Service, you will play a pivotal role in providing comprehensive care to clients referred from the Emergency Department and acute care inpatient units. Your responsibilities will encompass:

  • Assessment and Crisis Intervention: Conducting thorough assessments and providing immediate crisis intervention to individuals experiencing acute mental health challenges.
  • Brief Psychotherapy: Delivering focused and effective brief psychotherapy interventions to support clients in managing their immediate needs and developing coping strategies.
  • Disposition Planning: Collaborating with clients and the inter-professional team to develop and implement effective disposition plans, ensuring appropriate follow-up care and support.
  • Psychosocial Support and Psychoeducation: Offering vital psychosocial support and psychoeducation to clients and their families, empowering them with knowledge and resources to navigate their mental health journey.
  • Referral and Coordination: Facilitating referrals to both internal CAMH programs and external community services, ensuring clients have access to the resources they require for ongoing support.
  • Intake and Liaison: Managing intake calls from clients and families, gathering collateral information, and liaising with Access CAMH and other healthcare providers to coordinate seamless care.

This position within the acute care program CAMH demands a professional who is adept at working with diverse populations and thrives in a fast-paced clinical environment. Your expertise in behavioral observation, risk assessment, and crisis support will be essential. You will be expected to maintain meticulous documentation and foster effective communication within the inter-professional team. Job Requirements

To excel in this Social Worker position within CAMH’s acute care program, candidates must meet the following criteria:

  • Education: A Master’s of Social Work degree is mandatory.
  • Experience: Preference for 3 years of post-graduate experience in a mental health setting, particularly within crisis or acute care.
  • Registration: Registration and good standing with the Ontario College of Social Workers and Social Service Workers are essential. Eligibility for an adjunct faculty appointment with affiliated social work faculties is also required.
  • Clinical Expertise: A comprehensive understanding of DSM V diagnostic criteria and proficiency in various psychotherapy modalities, especially brief therapy models, are necessary. Knowledge of psychosocial rehabilitation principles, crisis intervention, case management, and recovery principles is crucial. Familiarity with emergency psychiatry and community resources for mental health and substance abuse treatment is highly valued.
  • Skills: Excellent organizational, interpersonal, and written communication skills are essential. Computer literacy, including word processing and electronic communication, is required. The ability to work collaboratively within an inter-professional team and demonstrate initiative is vital. Experience working with diverse clients and organizations is necessary. Bilingualism (French/English) or proficiency in another language is an asset.
