American Medical Response (AMR) is dedicated to providing exceptional and compassionate care to all patients. As part of this commitment, AMR ensures that patient medical data is handled with the utmost security and confidentiality. Understanding how to access your medical records is an important aspect of patient empowerment and informed healthcare decisions. This guide outlines the process for requesting your medical records from American Medical Response, ensuring you have the information you need while maintaining the highest standards of patient privacy and data protection, reflecting our compassionate care program.
To streamline and secure the process of medical record retrieval, American Medical Response has partnered with MediCopy, a specialized health information management company. All requests for medical records must be initiated through MediCopy’s online portal at medicopy.net/AMR. This partnership ensures accurate and timely delivery of your protected health information while adhering to all applicable legal requirements, reinforcing our commitment to responsible and compassionate patient care.
How to Request Your Medical Records:
Due to patient privacy regulations and to maintain the security of your personal health information, American Medical Response cannot process medical record requests over the phone. The online portal via MediCopy is the exclusive method for submitting your request. Patients are entitled to one complimentary copy of their medical records.
To begin your request, please follow these steps:
- Visit the MediCopy portal specifically designed for American Medical Response patients: medicopy.net/AMR.
- Complete the Medical Record Request form directly through the secure online portal. Ensure all fields are accurately filled out to avoid delays in processing your request.
Important Information Regarding Your Request:
- Delivery of Records: Once your request is processed, your medical records will be mailed to the address you provide on the request form. Please ensure your mailing address is current and accurate.
- Record Retention: American Medical Response retains medical records for a minimum of seven years, complying with all regulatory requirements.
- Authorization for Release: To protect patient confidentiality, AMR staff can only discuss patient information with authorized individuals. If you are requesting records on behalf of a patient, you must provide appropriate legal documentation:
- Patient Authorization: If you are not the patient, you must submit a signed authorization form from the patient allowing the release of their medical records to you.
- Power of Attorney: If you hold Power of Attorney for Healthcare Decisions for the patient, please upload a copy of the Power of Attorney document through the MediCopy portal at medicopy.net/AMR.
- Legal Guardianship: For requests concerning a child under 18 years of age, the child’s parent or legal guardian must complete the authorization form. For patients 18 years or older, they must complete the authorization themselves.
Special Document Requests:
American Medical Response understands that certain situations may require original documents such as affidavits or depositions. If your request necessitates a notarized citation, you should select mail delivery when submitting your request through MediCopy. This ensures the secure and legally sound delivery of these sensitive documents.
Processing Time and Follow-Up:
American Medical Response is committed to processing your request as efficiently as possible while maintaining accuracy and security. While state laws mandate a response within a specific timeframe (e.g., 15 days in Texas), AMR strives to complete all medical record requests within approximately 7 business days of receipt.
Upon submission of your request via the MediCopy portal, you will receive an estimated completion date. Due to the sensitive nature of medical records and the careful processing required for each request, we kindly ask that you refrain from status inquiries before this due date. This allows our team to focus on accurate and timely fulfillment of all requests, upholding our standards of compassionate and efficient service.
For further assistance or to follow up on a previously submitted request after the expected completion date, please contact MediCopy directly at 866.587.6274. MediCopy’s customer service team is available to assist you Monday through Friday, between 8:00 am and 4:00 pm Central Standard Time. They are equipped to provide updates and answer any questions you may have regarding your medical record request, ensuring a smooth and compassionate experience throughout the process.
