The Advanced Obstetrics Fellowship is excited about your interest in our program, now known as the Advanced Ob Care Program. Our core mission remains steadfast: to deliver sophisticated training in both high-risk and operative obstetrics to family physicians. We are particularly dedicated to those who are committed to serving in resource-limited environments and providing crucial care for systematically marginalized communities.
Program Goals
Our Advanced OB Care Program is structured around several key goals designed to enhance your skills and expertise:
- Comprehensive Labor Management: Develop proficiency in managing both uncomplicated and complex labor scenarios, ensuring patient safety and well-being throughout the birthing process.
- Operative Obstetrics Mastery: Achieve competency in operative obstetrics, encompassing cesarean sections and instrumented vaginal deliveries. This includes comprehensive post-operative patient care.
- Expert Prenatal Care for Complex Pregnancies: Deliver skilled prenatal care for pregnancies complicated by conditions such as diabetes, hypertension, thyroid disorders, substance use dependencies, and mental health challenges.
- Stabilization and Treatment Expertise: Gain the ability to effectively stabilize and treat pregnant patients who are hospitalized due to preterm labor, pre-eclampsia, diabetes-related complications, and placental abnormalities.
- Obstetric Ultrasound Proficiency: Attain competence in performing biophysical profiles and limited obstetric ultrasounds, essential diagnostic tools in modern obstetrics.
- Family Medicine Skill Retention: Maintain and enhance your existing family medicine clinical skills, ensuring a well-rounded approach to patient care.
- Obstetrics Teaching and Mentorship: Cultivate the skills necessary to effectively teach obstetrics to family medicine residents, contributing to the education of future physicians.
Program Objectives
To achieve these goals, the Advanced OB Care Program outlines specific objectives that fellows will accomplish:
- Efficient Obstetric Triage: Skillfully evaluate obstetric patients and interpret fetal monitoring tracings in triage settings for prompt and accurate assessments.
- Labor and Delivery Expertise: Provide expert labor management, facilitate vaginal deliveries, and perform instrumented deliveries for obstetric patients under the guidance of teaching panel physicians.
- Cesarean Section Surgical Skills: Perform cesarean sections as the primary surgeon, with progressive autonomy based on initial surgical skill level and under the mentorship of experienced obstetric providers.
- Gynecologic Surgery Exposure: Enhance surgical abilities by assisting in gynecologic surgeries, including tubal ligations and dilation and curettage (D&Cs), broadening surgical expertise.
- Postpartum Tubal Ligation Proficiency: Independently perform postpartum tubal ligations on patients within the teaching panel, providing a comprehensive range of obstetric services.
- Limited Obstetric Ultrasound Performance: Conduct limited obstetric ultrasounds during Labor and Delivery rotations, integrating diagnostic ultrasound into patient care.
- Resident Supervision and Preceptorship: Supervise family medicine residents in patient care through precepting at First Hill, Ballard, and Downtown Family Medicine residency clinics, fostering the next generation of physicians.
- Obstetrics Education for Residents: Educate Family Medicine residents on the obstetric service, covering crucial areas such as triage evaluation, cervical examinations, labor management, pitocin augmentation, intrauterine pressure catheter (IUPC) and fetal scalp electrode (FSE) application, vaginal delivery techniques, and postpartum care protocols.
- Didactic Teaching and Presentations: Lead didactic sessions on Labor and Delivery topics and contribute to Tuesday afternoon didactic sessions for Family Medicine residents, enhancing educational and presentation skills.
- Primary Care Obstetrics Presentation: Present on a relevant topic in primary care obstetrics at our annual High-Risk Obstetrics for the Family Physician CME (Continuing Medical Education) event, contributing to professional development and knowledge dissemination.
